Download the 2010 Buyer's Guide pdf and complete the form. You won't be able to save it, so print the form immediately after completing it or print to fill out by hand and submit it by fax or mail. If you choose to mail the form, please make sure to keep a copy for your records.
INSTRUCTIONS FOR DOWNLOADING AND MAILING/FAXING YOUR COMPLETED SURVEY TO APCO’s PUBLIC SAFETY COMMUNICATIONS
Step 1. Download the Buyer’s Guide form using the link above. You must have Adobe Acrobat Reader 6.0 or higher to complete the form. If you do not have Adobe Acrobat Reader 6.0 or later, please Download Adobe Acrobat Reader (free) or print the PDF and complete by hand.
Step 2. Open the downloaded .pdf survey in Adobe Acrobat Reader 6.0 or higher and complete the required fields. The form must be completed in one sitting. You cannot save it!

WHY CAN'T I SAVE FORMS COMPLETED IN ACROBAT?
Adobe Reader does not allow you to save completed forms. It only lets you print them. You must print your form before you quit the application. Otherwise, the data you entered will be lost. The only way to save a digital copy is to use: Acrobat Approval, Acrobat Standard, or Acrobat Professional.
